If you wish to gain access to a file/ information relating to a death where the investigation or inquest has been completed, please fill in this form in order for your request to be processed.

Please note information is only published on the approval of the coroner. Normally only interested persons have the right to access the case files unless there is written consent from the next of kin. Please note there may be processing costs involved.

Certain files are securely disposed of after 15 years. This approach is in accordance with Operational Selection Policy OSP6: Records created by and relating to Coroner 1970-2000 and has been approved by the National Archives.
